1.0
Purpose
Jefferson Lab cares about the welfare and productivity of its workforce and has several mechanisms for employees, or anyone who comes in contact with the Laboratory, to report concerns without fear of retaliation. In the case of issues potentially affecting the safety of workers, visitors, laboratory neighbors, or the environment, the process is designed to move rapidly towards eliminating the potentially hazardous situation. In all cases, information gathering and analysis is critical to prevent recurrence of the issue.

Scope
This chapter describes the options and processes for correcting hazards or dangerous situations at Jefferson Lab.
3.0
Responsibilities
3.1
Everyone
at Jefferson Lab
· If possible, resolve the ES&H concern yourself.
· Seek to resolve ES&H concerns through your supervisor and management chain using the mechanisms outlined in this chapter.
· Utilize your Division Safety Officer, your representative to the Worker Safety Committee, or contact ESH&Q directly if those resources would be helpful.
· Communicate any lessons learned to your staff, co-workers, supervisors and your Division Lessons Learned Coordinator.
· Utilize Jefferson Lab’s Employee Concerns Program (ECP) (http://www.jsaecp.ethicspoint.com) if you feel it would be more appropriate of if you would like to remain anonymous.
3.2 Supervisors & Managers
· Inform any staff expressing an ES&H concern of its initial disposition and periodically throughout any investigation.
· Advise any staff to utilize Jefferson Lab’s ECP (http://www.jsaecp.ethicspoint.com) if you feel it would be more appropriate.
3.3
ESH&Q
Staff
· When notified of an ES&H concern, decide whether it can be resolved informally or it should be reported to the Jefferson Lab ECP.
3.4 Associate Director - ESH&Q
· Provide resources and support to investigate and resolve all ES&H concerns
· Participate as required in Jefferson Lab’s ECP.
· Assure that appropriate materials are provided to the ESH&Q Reporting Officer.

ESH&Q
Reporting Officer
· Screen Jefferson Lab ECP reports for potential Occurrence Reporting or Noncompliance Tracking System reporting.
4.0
Expectations
What should you do if you spot a hazard or dangerous situation anywhere at Jefferson Lab?
4.1 Fix it yourself if possible, and you are properly trained, and understand the hazards.
4.2 Issue a Stop Work Order if you believe there is an imminent threat to people, health, the environment, or property.
4.3 If a resolution cannot be implemented, or if a Stop Work Order has been issued, immediately contact your supervisor.
4.4 You and your supervisor can then decide whether to involve higher levels of management, your Division Safety Officer, the appropriate Safety Wardens, or the ESH&Q Division.
4.5 When contacted for assistance to resolve a concern, ESH&Q staff will decide whether the concern should be resolved informally or should be forwarded to the Jefferson Lab ECP.
4.6 If informal resolution of a concern is ineffective or difficult to implement, it will be entered as a Jefferson Lab ECP.
4.7 Following the completion of investigations arising from ES&H related concerns reported through the Jefferson Lab ECP, the ESH&Q Reporting Officer will conduct appropriate reporting.
