Definitions, Guidelines & Requirements

Personnel Lift: Any lift involving a boatswain chair or personnel platform holding personnel.  When it is unavoidable, personnel may be lifted on a boatswain chair or platform using a crane if all criteria as laid down by OSHA in CFR 29 1926.550 are met, and a Temporary Operational Safety Procedure (TOSP) approved to cover the work.

 

Complex or unusual lift Any lift that:

·         Is outside the normal parameters expected of the group assigned to the work.

·         Is in a location with limited room to work and/or clearances for the equipment to operate.

 

Shall at the direction of the Group Leader assigned the task or the customer, require additional planning and documentation such as a Lift Plan or TOSP prior to starting work.

 

Potentially high-risk lift Any lift that:

·         Exceeds 90% of an onsite crane's rated capacity.

·         Due to its location poses an unacceptably high risk to personnel or equipment in the area of the operation as determined by the Group Leader, customer or supervisor of the team assigned the task.

·         Shall require at a minimum: Approval by the Material Handling Safety Representative (MHSR) and Group Leader of the lift plan and/or a TOSP.

·         The crane to be used shall, at the discretion of the MHSR or Material Handling Equipment Representative (MHER) be inspected (48-hour notice is required if an inspection is required).

 

Critical Lift Any lift that meets the criteria of a Complex, Unusual or Potentially High Risk Lift and/or:

·         Has two or more cranes working in unison or tandem to accomplish the lift.

·         Is deemed a Critical Lift by the Customer, Department Head, or Group Leader.

·         Involves high value equipment, components or facilities.

·         Would greatly impact the work schedule if material was damaged or destroyed.

·         Due to risk to the environment.

 

All such Critical Lifts shall require an approved, documented plan in the form of a TOSP, Operational Safety Procedure (OSP) or Standard Operating Procedure (SOP) and a lift plan.

 

All rigging equipment shall be inspected prior to use by the supervisor, PIC, and/or MHSR.

 

The crane to be used shall at the discretion of the MHSR or MHER, be inspected prior to use. (48-hour notice if an inspection is required)

 

Text Box: NOTE:  At the discretion of the customer, Group Leader and/or Department Head a “Critical Lift” can be downgraded to a lower classification if they believe that the Supervisor/SOTR's experience, knowledge and proven history does not require the level of documented planning laid down in this section.  These involved parties may set their own graded requirements as long as it is documented, risk based, and the SOTR/Supervisor agrees.  Guidance on risk assessment is in ES&H Manual Chapter 3210 Appendix T3 Risk Code Assignment.

 

Operators, Supervisors or SOTR’s shall notify the MHSR and obtain concurrence before operating a crane with any of the following situations:

·         Lifting personnel with a boatswain chair or platform

·         Complex or unusual lifts

·         Loads of uncertain weight that could exceed 50% of the crane's rated capacity

·         Loads that exceed 90% of the crane's rated capacity

·         Potentially high-risk lifts

·         “Critical lifts” must use a TOSP.  A TOSP is required to ensure that risks have been reduced to acceptable levels.

 

Crane Breakdowns In the event a crane breaks down with a suspended load, the operator:

·         Shall not leave a suspended load unattended.

·         Shall cordon off and label the area to prevent personnel from walking under the load.

·         Shall notify his/her supervisor and the MHSR and/or ESH&Q staff.

 

Crane Accident In the event of a crane accident:

·         The operator shall notify his/her supervisor, division ESH&Q staff on duty and/or the MHSR.

·         If the crane is damaged or possibly damaged it shall be locked out of service by one of the following:

o   The operator at the time of the accident,

o   The operator’s supervisor, and the person responsible for the crane, or

o   by the MHSR.

·         Notify MHER at ext. 7674, and the MHSR at ext 7406.

·         Secure the area around the damaged crane.

·         Call 911, if personal injury has occurred.

·         The crane shall be inspected by the MHER, MHSR, or a qualified person to evaluate any damage.

·         The crane shall NOT be returned to service until repaired, and if applicable, acceptance testing has been completed and approved by the MHER or MHSR.
