Work Planning and Execution for Electronic Equipment Safety

 

 

1.0             Purpose

 

The work rules for electronic equipment are dependent on both equipment Class and work Mode. Workers must determine the training requirements and applicable work rules once the equipment Class and work Mode are identified. The required steps for turning a work assignment into a work plan can then be determined.

 

2.0             Scope

 

Work planning requires a defined scope, a hazard analysis, and development of hazard controls.

 

This appendix provides an outline for determining the steps necessary to plan work on this type of equipment, identify the hazards, and develop work controls. General work guidance for all electronic equipment work is also included.

 

3.0             Responsibilities

NOTE:           Management authority may be delegated to a task qualified Jefferson Lab employee at the discretion of the responsible manager.

 

3.1       Qualified Electronic Worker

·         Understand work scope and safety requirements.

·         Ensure appropriate training and the qualifications for assigned work is current.

·         Prepare and submit work plans, which include completing a Job Briefing checklist, hazard identification and mitigation requirements, and conducting a job briefing with all employees involved.

·         Follow approved work plan as developed.

·         If the work will carry over multiple work shifts, hang the job briefing checklist in a conspicuous location at the work site.

·         Stop work if required when the work deviates from work plan. Revise the work plan including changes in safety requirements as necessary. Get approval as necessary for revised work plan.

·         Provide feedback on work completed for lessons learned.

 

3.2       Supervisor/Subcontracting Officer’s Technical Representative (SOTR)/Sponsor

·         Assign work to qualified and trained personnel.

·         Prepare, review, approve, and communicate work plans.

·         Ensure all necessary Work Control Documents are in place and current.

·         Provide work oversight as necessary.

 

4.0             Process Steps

 

The general outline for all work planning and execution can be found in Work Controls and Processes Procedure Diagram. Work on electronic equipment follows the steps contained in the flow chart.

 

4.1              Hazard Avoidance and General Guidelines

At a minimum, the following items are considered in the preparation and execution of work on electronic equipment. They are starting points in preparing for work and for avoiding electrical hazards.

·         Whenever possible work with equipment de-energized.

·         Always work at the lowest Class and Mode consistent with accomplishing the work.

·         Be sure you have the latest documentation and understand the current work rules.  Review before beginning work.

·         Be sure all test equipment is in good working condition and is rated for the testing to be done.

·         Observe all locks and tags, warning signs, and labels on equipment being serviced.

·         Know the Limited and Restricted shock approach boundaries and the arc flash boundary when performing work on energized equipment.

·         Make no modifications to equipment unless you are qualified and authorized. For more information see ES&H Manual Chapter 6240 Electronic Equipment – Construction and Modification Safety.

·         Maintain safe distances from exposed electrical wiring unless required to approach to accomplish work.

·         Recognize that equipment combining high voltage (>10kV) and vacuum may be a source of X-rays.

·         When practicable, follow the good practice of having your work checked by another qualified person before restoring equipment to service.

·         Always bring suspected hazards or off-normal situations to the attention of your supervisor

 

4.2              Work Planning and Execution:

The following steps are followed when developing and executing a work plan for electronic equipment. Not all steps are required for each job. Applicability is based on the details of each specific job.

·         Identify task.

·         Determine equipment Class and work Mode (ES&H Manual Chapter 6230 Appendix T1 Determining Equipment Class and Work Mode).

·         Determine worker qualification, and rules required for the task.

·         Verify worker qualifications then assign work.

·         Collect and review applicable Work Control Documents, equipment documentation, lessons learned, and equipment specific procedures.

·         Identify approach boundaries as necessary for energized work.

·         Analyze all hazards and determine mitigation.

·         Determine personal protective equipment (PPE) and other equipment requirements.

·         Develop work plan, and write Work Control Documents as necessary

·         Generate work planning task list (ATLis, FEList, etc.).

·         Receive approval to do the work.

·         Conduct pre-job briefing/job walk-down.

·         Perform work as planned remaining within the envelope of the work plan.

·         Check work prior to re-energizing or restoring equipment. 

·         Document work if required.

·         Write lessons learned, if applicable, and close task list.
