- Subcontractor lacerated finger requiring stitches (ORPS/CAIRS)
- Subcontractor fell and sprained knee (ORPS/CAIRS)

- 3 were hand/finger injuries
Summary
- We have had 9 Recordable injuries in the 1st and 2nd quarters of the FY. 3 of the 10 were DART injuries. Based on these data we not will meet our TRC and DART goals this year.
- Subcontractor employees account for half of our recordable injuries but JLab employees suffered all 3 DART injuries.
- About 82% of "Unsafe" Safety Observations relate to subcontractor observations in Q212.
- PPE safety issues have risen during the 2nd quarter. From January through March:
- 48 PPE related unsafe observations were reported.
- 35 unsafe observations related to tools and equipment were reported
- The focus on ladders is bringing related non-conformances down.
- Issues related to electrical cords and extension cords have dropped due to attention paid to this issue
