Administrative Manual - 205 Benefits

205.01 Benefits Program Overview

  1. POLICY

    It is the policy of JSA/JLab to provide a benefits program for eligible regular and term employees that offers coverage for health and welfare, life and disability insurance, and retirement needs.

    All premiums, limits of coverage and provisions of coverage shall be governed by the plan document currently in force. In the event of any contradiction between information appearing in this manual and the master contract or plan documents the latter shall govern in all cases.

    JSA/JLab reserves the right to amend or terminate any of these programs, or to require an increase in employee premium contributions toward any benefit, at its discretion.

  3. ELIGIBILITY
    1. Except where otherwise noted, regular and term staff members normally scheduled to work at least 50% time or more are eligible to participate in JSA/JLab benefits programs as of date of hire or transition to an eligible status from an ineligible one.
    2. Dependent eligibility for coverage under the employee's plan is limited to spouse and children. Coverage, or its continuation, for children may vary from plan to plan.
  4. ELECTION CHANGES
    1. After initial enrollment, the Lab has annual open enrollment periods for selected benefits during which choices must be reaffirmed or may be changed. Generally benefits can be changed during the year only when there is a family status change such as through marriage, legal separation, divorce, death, birth, adoption or spousal job change. This change must be completed within thirty-one (31) days of the event. For some plans, if notification is not timely, evidence of insurability may be required.
    2. Responsibility to initiate timely enrollment actions or failure to do so, with subsequent loss of benefits, rests with the employee.
  5. SUMMARY PLAN DESCRIPTION

    A summary plan description shall be made available to all employees for a full description of specific benefit plans. These
    summaries include information on how to appeal decisions that have been made by plan carriers or sponsors related to denial of benefits.

  6. TERMINATION OF COVERAGE

    Benefits either terminate at midnight on the day of the employee's termination of employment, transition to an ineligible status, or at other specified time frames. Select benefits, due to pre-payment of premiums, extend to the end of the month worked.

  7. CONTINUATION OF COVERAGE

    Coverage after separation is available for some insurance plans if acted upon within prescribed timeframes. Employees must contact Human Resources/Benefits staff for information regarding eligibility to extend insurance benefits.

  8. SUMMARY TABLE OF PLANS

    Exhibit 205.01 shows JSA/JLab benefits and eligibility by employment status or hours worked. Benefits such as Educational Assistance, paid leave plans such as vacation, holiday, and sick leave, are covered in other sections of the Human Resources and Services Section of the Administrative Manual.
