E89-019 and E94-012 Shift Request Form

Guidelines are for 5 shifts minimum / 10 shifts desired over the course of the experiment.
The experiment will begin on August 7^th, 1999 and end on November 4^th, 1999. Two long maintenace periods split it into three run periods:
- August 7 to August 22 (August 23 - 31 are to be determined.)
- September 14 to October 2
- October 13 to November 6
- Only target operators should sign up for the 3-day maintenance times: September 21 - 23 and October 25 - 27.
You may check shift availability before submitting your dates.

If you are contributing other than by running shifts, please specify below, and indicate available dates if appropriate.
