Permit-Required Confined Space (PRCS) Identification and Signage

 

 

At Jefferson Lab a Confined Space Entry (CSE) Permit is considered to be a Work Control Document and thus a Standard Protecting Measure.  Only when all hazard issues associated with the PRCS and the work activity are acknowledged and mitigated, in accordance with this program, can the area’s associated Risk Code be considered < 2.  At a minimum, all work requiring entry into a PRCS is discussed with the entry supervisor prior to commencement. 

 

A formal Task Hazard Analysis and Operational Safety Procedure is required for work considered to be Risk Code >2 in accordance with ES&H Manual Chapter 3210 Work Planning, Control, and Authorization Process.

 

1.0            Purpose

 

Jefferson Lab identifies areas that qualify as Permit-Required Confined Space (PRCS) using the Facility Design and Modification Review (see ES&H Manual Chapter 3110 Facility Design and Modification Review).  This appendix describes:

 

·         Typical area signage; and

·         How to update, repair and maintain signage.

 

2.0            Scope

 

This appendix does not apply to sewer manholes, wastewater pump pits, or similar areas.  For information on posting and entry requirements for these areas contact the Confined Space Coordinator.

 

The process steps for this procedure are performed in coordination with ES&H Manual Chapter 6160 Permit-Required Confined Space Entry Program.

 

3.0            Responsibilities

NOTE:           Management authority may be delegated at the discretion of the responsible manager.

 

3.1              Anyone at Jefferson Lab

·         Request an area hazard evaluation from Industrial Hygiene for areas that meet or exceed the criteria of PRCS and is not properly identified. 

 

3.2              Supervisor/Subcontracting Officer’s Technical Representative (SOTR)/Sponsor

·         Know the PRCSs under your authority ensure they are appropriately identified and secured.

 

3.3              Industrial Hygiene

·         Perform a hazard evaluation and provide appropriate signage as required.

·         Provide OSHA-prescribed confined space danger signs as requested.

 

4.0            Process Steps

 

Unless otherwise stated, anyone at Jefferson Lab may perform the process steps outlined below.

 

4.1              Area Posting Requirements:

 

4.1.1        PRCSs

The following signage or similar is posted at the entrances to all PRCSs:

 

 

Entry into an area so labeled requires an approved Confined Space Entry Permit.  (See ES&H Manual Chapter 6160 Appendix T3 Permit-Required Confined Space Entry Permit.)

 

4.2              Update, Repair, or Maintenance to Postings 

Anyone at Jefferson Lab may request new, update, repair, or maintenance to a posting for a PRCS, or confined space.

 

·         Contact Industrial Hygiene and state the posting or area needing review.

o   If it is determined that the label is defective, the Industrial Hygiene will rectify the situation.

o   If no hazard is found – Industrial Hygiene informs the concerned party(s) of the determination.
