Work-Related Injuries Occuring While on Travel

Occupational Medicine would like to provide information to you in the event that you experience a work-related injury while you are on Jefferson Lab-related travel. We hope that you will not need to use this information, but if you are injured we would like to help you to obtain appropriate treatment as quickly as possible.

Please keep this information with you while you are away from the Lab. If you are injured, please follow these instructions:

  • Obtain treatment for the injury. In case of an emergency, go to the closest hospital emergency department. For less serious injuries, if you are at another DOE facility you may obtain treatment from the medical department at that site. If you are not at a DOE facility, use the nearest urgent care center or emergency department to obtain treatment. Be sure to tell the treating facility that this is a work-related injury.
     
    • Jefferson Lab's workers' compensation insurance carrier is The Hartford and our policy number is 14WBTK8734. All claims should be mailed to Jefferson Lab Occupational Medicine, MailStop 28C, 12000 Jefferson Ave., Newport News, VA, 23606.
  • Contact Occupational Medicine as soon as possible after obtaining treatment. Please call the nurses at (757)269-7539
  • Notify your supervisor.
  • Upon your return to the Lab, report to Occupational Medicine as soon as possible. Bring any medical bills, insurance claim forms, prescription receipts or work notes with you at that time.
