205.04 Life Insurance Plans

JSA/JLab provides an employer paid life insurance plan and an employee paid supplemental life insurance plan to eligible employees.

  1. GROUP TERM LIFE INSURANCE
    1. Policy

      JSA/JLab provides a group term life insurance plan at no cost to the employee. Participation in the plan is automatic.

    2. Eligibility
      1. All regular and term employees who are normally scheduled to work at least 50% time are eligible upon employment or date of transition to eligible status. Eligible dependents covered under this policy include:
        1. Spouse, unless legally separated,
        2. Unmarried children under the age of 25, living with the employee and dependent on the employee for support and maintenance, unmarried, and not employed full-time.
      2. Employees on Inactive-Disability status (see Policy 205.01 D.) will have their life insurance premium paid by the Lab for a period of up to 13 months until a decision is finalized on waiver of premiums by the insurance carrier.
    3. Premiums

      The cost of this insurance is paid entirely by JSA/JLab.

    4. Plan Provisions
      1. The participant is covered by a life insurance benefit equal to twice the annual base salary rounded to the nearest $1000 for a maximum amount of $350,000. There is also a double indemnity provision through Accidental Death and Dismemberment Insurance in an equivalent amount, which pays when death is the result of accidental injury or there has been serious injury. There is an accelerated benefit in the event of a loss of sight, speech, hearing or a limb. Benefits are paid to the designated beneficiary.
      2. The plan also contains dependent life insurance, which provides a death benefit to the employee for the death of a spouse or a dependent child.
      3. Coverage ends when the employee is no longer in an eligible employment status or terminates employment with the Laboratory, or the group policy is terminated.
      4. When an unmarried dependent becomes ineligible because of age, coverage can be extended if the dependent cannot earn a living due to mental or physical handicap. Application must be initiated by the employee participant.
      5. Upon exit from eligible status or employment, the employee may convert to an individual insurance policy without submitting evidence of insurability if the option is exercised within 31 days from change of status. Insured dependents may also convert.
  2. SUPPLEMENTAL LIFE INSURANCE
    1. Policy

      JSA/JLab provides additional life insurance coverage to eligible employees and their dependents. Participation is optional.

    2. Eligibility

      All regular and term employees who are normally scheduled to work at least 50% time are eligible upon employment or date of transition to an eligible employment status. If enrollment is not elected at date of eligibility, enrollment is contingent on the carrier accepting medical evidence of good health.

    3. Plan Provisions
      1. The supplemental life insurance offered is a term life insurance product that provides protection for the employee and his/her family. At time of hire, and during open enrollment, the employee may obtain this additional coverage upon approval of the carrier.
      2. When an employee terminates from the Lab, the term insurance is portable. Coverage may be continued in effect by paying premiums directly to the carrier at a rate determined by the carrier.
    4. Premiums

      The premium cost is paid entirely by the employee.
