    Requirements for Access to Jefferson Lab

    Jefferson Lab is subject to the Department of Energy's current policy on Unclassified Foreign Visits and Assignments. The policy requires the Lab to check identification and US Citizenship & Immigration Services (USCIS) documents.

    To comply with this policy, and to expedite your on-site registration, please send us prior to the workshop the following information:

    1. Country of citizenship
    2. Country of birth
    3. Affiliation (employer or organization)

    If you are a U.S. citizen, and do not possess a valid DOE or DOE-laboratory ID badge, you will be asked to present other photo identification (passport, government-issued ID, or valid driver's license) when you arrive.

    If you are not a U.S. citizen, and do not possess a valid Jefferson Lab ID badge, you will be asked to present the following (as applicable) when you arrive:

    • Original passport
    • Form I-94 (not required for Canadian citizens)
    • Applicable visa
    • USCIS documentation supporting the visa status renewals (e.g., form I-20 for F-1 students or form IAP-66/DS2019 for J-1 visa holders

    PLEASE NOTE: This information must be provided in full or you may not be permitted access to Jefferson Lab. It is important that you reply to bizot@jlab.org as soon as possible and no later than close of business Friday April 9, 2004.
