1.0 Introduction

There are more than 100 spaces at Jefferson Lab which are considered to be permit-required confined spaces (PRCSs) or must be treated as such until qualified staff determine that there are no actual or potential hazards. A few of the PRCSs are located inside buildings, and Jefferson Lab employees or subcontractors make infrequent entry – typically for inspection or maintenance. The majority of PRCSs are underground, are entered through manholes, and were built to protect and allow access to a single utility, such as electrical power, communications systems, and sanitary sewer. Entry into these spaces is also infrequent and normally is made by representatives of the utility or a Jefferson Science Associates, LLC (JSA) subcontractor.

Jefferson Lab’s official list of PRCSs is maintained as Jefferson Lab drawings 00-C-8-2142-001 and 00-C-8-2143-002 “Confined Space Locations.”

PRCSs are listed in this appendix to illustrate their number, types, and locations at Jefferson Lab. This list was current on the date of issue of this document.

2.0 List of Permit-Required Confined Spaces

Every PRCS is assigned a unique alphanumeric designation. The assignment of the designator is based on the following system:

Communications manholes	EMHC followed by consecutive numbers starting with 1
Electrical power manholes	EMH followed by consecutive numbers starting with 1
Lift station-sanitary sewer	LS followed by consecutive numbers starting with 1
Lift station-storm water	LW followed by consecutive numbers starting with 1
Mechanical manholes	MMH followed by consecutive numbers starting with 1
Sanitary Sewer manholes	SSMH followed by consecutive numbers starting with 1
Water Meter Vault	W followed by consecutive numbers starting with 1
Other kinds of Vault	V followed by consecutive numbers starting with 1
Unverified means precise location or hazard potential has not been fully verified by Laboratory ESH&Q staff.
