Appendix T2 Hazard
Issues Update Procedure |

Identification of federal, state, local, and contractual
requirements (hereafter known as “regulations”) associated with hazard issues at Jefferson Lab is
of utmost importance. This procedure
describes the process used to identify hazard
issues and maintain the procedures within this manual
to comply with relevant regulations and best practices.
Jefferson Lab’s identified hazard issues are listed in ES&H Manual Chapter 2410 Appendix T1 Hazard Issues List. These issues are identified by Division Safety Officers, and Subject Matter Experts (SME); and acknowledged by upper level management.
This procedure outlines how:
· Hazards are identified, added, or deleted;
· The Hazard Issues List is updated; and
· Changes are communicated to the laboratory population.
NOTE: Management authority may be delegated at the discretion of the responsible manager.
· Identify new hazard issues that may need to be addressed and bring them to the attention of your Supervisor.
· Bring any new hazard issues to the attention of your DSO.
3.3 Division Safety Officers (DSO)
· Bring any new hazard issues to the attention of the Associate Director – ESH&Q.
3.4 Associate Director – ESH&Q
· Confirm hazard issues.
· Ensure appropriate regulations are identified on the Hazard Issues List.
· Ensure ES&H Manual content is updated appropriately.

4.1
Use
of Existing Hazard Issues
Recognized
hazard issues are located in ES&H Manual Chapter
2410 Appendix T1 Hazard Issues List.
Individuals needing guidance regarding a hazard issue:
· Search ES&H Manual Chapter 2410 Appendix T1 Hazard Issues List for the hazard.
· Review the related ES&H Manual content.
· Contact the relevant SME for additional guidance.
· Contact Division DSO if guidance does not adequately address the hazard.
4.2 Update Hazard Issue List
If a hazard issue is not currently covered within the ES&H Manual
Step 1: Anyone at Jefferson Lab is to bring a newly recognized hazard issue to the attention of their Supervisor, thence to the DSO. The DSO then brings the issue to the Associate Director – ESH&Q.
Step 2: The Associate Director – ESH&Q makes the following determination:

4.3 Removing a Hazard Issue from the List
Step 1: SME notifies Associate Director – ESH&Q that a hazard is no longer relevant to Jefferson Lab.
Step 2: Determination is made by the Associate Director – ESH&Q to remove the content from ES&H Manual Chapter 2410 Appendix T1 Hazard Issues List.
Step 3: Hazard Issues are deleted in accordance with ES&H Manual Chapter 1300 Appendix T1 Content Review Procedure.
